It all depends on the campground, not all KOA's do it, and I've seen other private campgrounds do it. Some charge for the 2 adults than each child is extra, some charge for a family of four and any extra people is a charge etc. I have never seen public campgrounds do it, but like said before, thay have a certain number per site. Fort Wilderness at Disney charges per site and you can have up to 10 people per site. They are all different.
